Social workers play a unique role on the interdisciplinary care team.  In best practice case management models the social worker focuses on the most psychosocially complex patient and family caregivers and provides them with psychosocial support, interventions and assistance with discharge planning as needed. This program will review the ways in which a social worker can organize their daily schedule using the case management process.  We will review each step in the process from the social work point of view as well as how the social worker can engage with RN case managers and the interdisciplinary care team.  This will be followed by the Conditions of Participation for Discharge Planning, referral to home care and working with patients and family caregivers in a compliant environment.  A detailed review of the role of the family caregiver will complete this program.
